<h1><span data-text-color="primary">Authentic Assessment</span>: A Practical Guide for <span data-text-color="primary">Educators</span></h1>

<p>For many years, medical education has demonstrated that authentic learning environments and real-world learning tasks lead to strong learning outcomes. This is especially true in programs where entrustment plays a central role — where learners must demonstrate that they can be trusted to perform professional responsibilities independently.</p>
<p>In healthcare education, skills must be practiced repeatedly, deeply internalized, and demonstrably mastered.</p>
<p>For a long time, this approach was described as holistic education or holistic assessment. The content framework was often built around Entrustable Professional Activities (EPAs) or competencies, while the assessment framework followed principles of programmatic assessment.</p>
<p>In recent years, this model has increasingly been referred to as authentic learning and authentic assessment.</p>
<h3><span data-text-color="primary">What Is an Authentic Learning Environment?</span></h3>
<p>Australian researchers Jan and Anne Herrington conducted extensive research on authentic learning environments. Drawing on both theoretical and practical insights, they identified key characteristics: learning should be student-centered, realistic, and effective.</p>
<p>Authentic learning moves away from the traditional model of large lecture halls where experts transfer knowledge in small, pre-packaged segments to passive students. Instead, it reflects how learning actually takes place in professional practice.</p>
<p>In their article Authentic Learning Environments in Higher Education (2006), they describe nine essential elements:</p>
<ul>
<li>Authentic context</li>
<li>Authentic activities</li>
<li>Access to expert performance and modeling</li>
<li>Multiple perspectives</li>
<li>Collaboration</li>
<li>Reflection</li>
<li>Articulation of learning</li>
<li>Coaching and scaffolding</li>
<li>Authentic assessment</li>
</ul>
<p>These elements create a learning experience that mirrors professional reality.</p>

<h3><span data-text-color="primary">Generative AI and Authentic Learning</span></h3>
<p>To date, authentic learning has been most widely implemented in medical and allied health education. However, the rise of Generative AI (GenAI) is accelerating broader adoption.</p>
<p>Traditional assessment methods are under pressure. As AI tools make it easier to generate written responses or complete standardized tasks, education must shift toward forms of assessment that evaluate what learners can actually do in real-world contexts.</p>
<p>Authentic assessment provides that solution.</p>
<p>This does not mean GenAI and authentic learning are incompatible. On the contrary, AI can support processes such as:</p>
<ul>
<li>Summarizing feedback</li>
<li>Structuring recorded case discussions</li>
<li>Analyzing dashboard data</li>
<li>Supporting formative feedback processes</li>
</ul>
<p>Used thoughtfully, AI can save time and support both learners and faculty.</p>
<p>At the same time, institutions are debating issues such as data security, ethical use, intellectual ownership, and the broader cognitive impact of AI. GenAI is not flawless. It can hallucinate, fabricate references, and produce inaccurate outputs. These limitations are widely acknowledged.</p>
<p>More importantly, authentic learning depends on reflection and articulation. These core learning processes cannot be outsourced to AI. They require human effort and cognitive engagement.</p>

<p>Educational programs increasingly recognize that knowledge-based testing alone no longer delivers sufficient learning impact.</p>
<p>Research shows that active engagement leads to stronger learning outcomes. Authentic contexts increase student involvement and ownership of learning. That shift requires a different approach to assessment.</p>
<ul>
<li>Authentic assessment often includes:</li>
<li>Open-ended tasks</li>
<li>Multiple possible solutions</li>
<li>Diverse pathways to competence</li>
<li>Context-rich assignments</li>
</ul>
<p>Theory without context has limited meaning. Real tasks in real environments create depth and integration. Fragmented learning objects often reduce complexity and weaken understanding.</p>
<p>As Aristotle observed long ago: the whole is greater than the sum of its parts.</p>
<p>Authentic learning integrates:</p>
<ul>
<li>Theoretical knowledge</li>
<li>Practical skills</li>
<li>Professional attitude and judgment</li>
</ul>
<p>Medical education has been applying this integrated approach for years, and other disciplines are increasingly recognizing its value.</p>

<p>Authentic assessment is difficult to implement without a supporting portfolio system. Assessment methods must align with the type of learning activity. This may include:</p>

<li>Reflections</li>
<li>Workplace-based observations</li>
<li>Knowledge assessments</li>
<li>Multi-source feedback</li>
</ul>
<p>Learners must be able to reflect on their experiences, and feedback plays a central role. Assessment may also involve multiple assessors and multiple data sources. These data points collectively form a rich body of evidence that can be evaluated within a portfolio framework. A portfolio systematically collects evidence of performance, including:</p>
<ul>
<li>Observations</li>
<li>Assessments</li>
<li>Feedback</li>
<li>Self-reflections</li>
</ul>
<p>Visualizing this evidence through dashboards supports learner ownership and enables supervisors to monitor progress. Longitudinal tracking is particularly valuable for identifying patterns in development over time.</p>
<h3><span data-text-color="primary">Choosing the Right Portfolio</span></h3>
<p>When selecting a portfolio platform for authentic learning, several factors are critical:</p>
<ul>
<li>It must support individual learning pathways</li>
<li>It should enable a wide variety of assessment processes</li>
<li>It must not disrupt learning, but support it through intuitive design</li>
<li>It should allow external workplace assessors to provide authenticated feedback without requiring full system accounts</li>
<li>It must ensure data security and privacy</li>
</ul>
<p>Authentic learning focuses primarily on the individual learner, even when certain assessments (such as OSCEs or peer evaluations) take place in group settings. Data must always be stored and visualized at the individual level.</p>
<p>A well-designed portfolio increases reliability without adding administrative burden.</p>
<h3><span data-text-color="primary">Conclusion</span></h3>
<p>Authentic assessment aligns education with professional reality. It strengthens engagement, integrates knowledge and skills, and supports meaningful reflection.</p>
<p>In an era shaped by AI and rapidly changing professional demands, authentic assessment is not just an innovation — it is a necessity.</p>

<h3><span data-text-color="primary">Further development of portfolios and integration of EPAs</span></h3>
<p>Throughout the project, the participating programs continued to refine their portfolio-based assessments and align them with their curricular requirements. New competency models were integrated and formative feedback processes strengthened.</p>
<p>A particular focus was the introduction of <span data-text-color="primary"><a href="https://scorion.com/en/what-are-epas-and-how-do-you-assess-them/" target="_blank" rel="noopener">Entrustable Professional Activities (EPAs)</a></span> in the portfolios. The programs used Scorion to structure EPAs, make feedback across clinical situations visible, and support individual learning pathways more precisely. This helped increase transparency in learning development and enabled more structured documentation of practice-oriented competencies.</p>
<h3><span data-text-color="primary">Collaboration within the project</span></h3>
<p>More than 40 researchers and educators from various disciplines were involved in the HEDS project. Working with this open, dedicated and interprofessional team was especially valuable for us.</p>

<h1>AI in <span data-text-color="primary">Medical Education</span> Assessment: A Pilot</h1>
<p>Eduface and Scorion are collaborating with a leading university on an innovative pilot exploring how artificial intelligence (AI) can enhance assessment processes in medical simulation-based education.<br />The goal is to explore how AI can support educators in structuring and processing feedback within Scorion.</p>
<h3><span data-text-color="primary">Current Assessment and Feedback Workflow</span></h3>
<p>In simulation training, students complete an assessment form in Scorion before the session. During the simulation, they receive verbal feedback from both the instructor and the simulated patient. The instructor then makes informal notes of this feedback outside Scorion, later refining and manually entering it into the Scorion form. Once validated, the feedback becomes visible to the student.</p>
<p>This process is valuable but time-consuming. The pilot explores whether AI can streamline this workflow without compromising quality or educational value.</p>
<h3><span data-text-color="primary">How AI Can Improve the Process</span></h3>
<p><span style="font-size: 14.4px;">The university sees potential for AI to convert draft notes into structured feedback mapped to the correct Scorion fields. Together with Eduface and Scorion, a structured format is developed focusing on the competency domains Communication and Professionalism, addressing key questions such as:</span></p>
<p>What is going well?</p>
<p>What could be improved?</p>
<p>AI can help make feedback more consistent and faster to process, while educators retain full control over content and interpretation.</p>
<h3><span data-text-color="primary">The Test Run: First Step Toward AI Integration</span></h3>
<p>To determine whether an AI model can be effectively trained, a test run is being conducted within Eduface – initially without direct integration into Scorion. This test will show whether AI can reliably and educationally responsibly structure feedback.</p>
<p>The test run includes five phases:</p>
<ul>
<li>Data collection: the university provides 10 draft versions and 10 completed Scorion forms.</li>
<li>Conceptualisation: analysing what defines well-structured feedback.</li>
<li>Data optimisation: expanding and refining the dataset for model consistency.</li>
<li>Initial training and interim validation.</li>
<li>Final validation: 90-day testing period.</li>
</ul>
<h3><span data-text-color="primary">The Future of AI in Medical Education Assessment</span></h3>
<p>Through this pilot, <a href="https://www.eduface.me/" target="_blank" rel="noopener">Eduface</a>, Scorion, and the university aim to explore how AI can make assessment and feedback in medical education smarter, more consistent, and more efficient – while maintaining quality and the personal value of feedback.</p>

<h1>QR-Code function in Scorion: <span data-text-color="primary">fast feedback.</span></h1>
<p data-start="723" data-end="910">Giving digital feedback has never been easier. And the QR code feature in Scorion is almost ready to go. We’re currently making a few final adjustments to ensure everything runs smoothly.</p>
<p data-start="912" data-end="1136">Scanning a QR code <span data-text-color="primary"><a href="https://youtu.be/oWgCLenOcec" target="_blank" rel="noopener">(video on YouTube)</a></span> to submit an assessment or provide feedback is currently being thoroughly tested and piloted at a university. This incredibly practical feature is expected to be available in early 2026.</p>
<p data-start="1138" data-end="1250">In short: from that point on, every user will be able to give feedback even faster using the QR code in Scorion.</p>

<p data-start="1566" data-end="1894">A key part of programmatic assessment and working with EPAs is collecting data points. These data points can include various assessment formats — such as reflection, peer feedback, workplace-based assessment, or the submission of an assignment by the student. With the QR code, this process can be shortened by up to 90 percent.</p>
<h3 data-start="1896" data-end="1932"><span data-text-color="primary">Workplace Based Assessment (WBA)</span></h3>
<p data-start="1934" data-end="2201">This is especially noticeable in clinical or workplace settings. Until now, students had to send an assessment or feedback request to an evaluator or instructor. With the QR code, students can open the form directly in the app on-site and have it scanned immediately.</p>
<h3 data-start="2203" data-end="2247"><span data-text-color="primary">Simple feature, significant time savings</span></h3>
<p data-start="2249" data-end="2673">Training programs often want to know precisely how a student is progressing — and students usually want that too. Especially in programs where entrustment plays a key role, “roughly” is not good enough. Educators need reliable information to determine whether a trainee physician is ready for a high-risk procedure on a real patient. That requires dependable data and a fast workflow. With the QR code, this is now possible.</p>

<p style="text-align: left;">In recent years, there has been a justified increase in attention to <strong>inclusivity within medical education</strong>. A diverse student population and an inclusive learning environment are essential for training doctors who can effectively deal with a diverse patient population <sup>(1,3)</sup><span class="whitespace-nowrap">.</span> About 20 years ago, the physician population was predominantly male and white. Since then, significant progress has been made in the ratio of men to women. Among all doctors, the number of women has grown from 32% in 2005 to 46% in 2016. In 2016, there were not yet more female doctors than male, but now there are across all doctors. Among specialists, the increase was from 26% in 2004 to 41% in 2016. The percentages vary considerably within specialties: cardiac surgeons and orthopedists were still predominantly men in 2016, at 11% and 12% respectively. And this ratio has not changed much yet.</p>
<p style="text-align: left;">Among general practitioners, this is the case, according to (Dutch) figures from the CBS. We see that the number of female GPs is now greater than the number of male GPs.</p>

<p style="text-align: left;">Although considerable steps have been taken in the ratio between men and women, there is still much room for improvement. Currently, the number of medical specialists with a migration background is estimated to be between 2 and 4 percent<span class="whitespace-nowrap">.</span>There is still much work to be done if the physician population is to become a reflection of our society. This work begins in the training programs. And although we should not overestimate the contribution and role that digital systems can play in this, we do think that a portfolio tool can help. In this article, we explore the current state of affairs and how digital tools such as e-portfolios can contribute to more inclusive medical education.</p>

<h3><span data-text-color="primary">The challenges of inclusivity in medical education</span></h3>
<p>At the 2024 conference of the NVMO (Dutch Association of Medical Education) with the theme &#8216;Equal Opportunities in Healthcare Education&#8217;, inclusivity was the main theme of the conference with a full program line on this topic. This shows that there is certainly attention to the subject within education. Yet much research also shows that inequality still exists in access to medical education. Students from affluent families or with parents in the medical sector have a greater chance of being admitted, while students with a migration background are underrepresented <sup>(3)</sup><span class="whitespace-nowrap">.</span> This lack of diversity not only has consequences for the students themselves but also for the quality of healthcare as a whole.</p>
<p>In addition to the selection procedures, experiences during the training also pose a challenge. Students from minority groups regularly report experiences with aggressions and subtle forms of discrimination <sup>(3)</sup><span class="whitespace-nowrap">.</span> These experiences can have a negative impact on their well-being and performance.</p>

<h3><span data-text-color="primary">Towards a more inclusive curriculum</span></h3>
<p>Research worldwide also shows that diversity within medicine improves its quality. And when we see that there is still a considerable imbalance between the physician population and society, there is still a lot to be gained<span class="whitespace-nowrap">.</span>One of the studies recently conducted at the GP training programs has formulated a so-called DEI (Diversity, Equity, and Inclusivity) strategy. This consists of 7 very concrete strategies, of which these are the most important:</p>
<ul class="marker:text-textOff list-disc pl-8">
<li>DEI expertise promotion for all those involved in GP training using training programs;</li>
<li>Appointment of DEI ambassadors in all teams of GP training, who contribute to knowledge transfer, reflection on possible prejudices;</li>
<li>Participation in DEI networks; Giving an active voice to trainees from ethnic minority groups in developments within GP training.</li>
</ul>
<p>Despite the fact that there is a lot of attention and a number of initiatives have already been started within medicine, there are still plenty of challenges remaining. To address these challenges, it is essential that medical education actively works towards a more inclusive curriculum. In general, the following aspects are often mentioned as supporting the process towards more &#8216;DEI&#8217;:</p>
<ul class="marker:text-textOff list-disc pl-8">
<li>Awareness of implicit biases</li>
<li>Knowledge about health issues specific to particular groups</li>
<li>Skills to communicate effectively with a diverse patient population</li>
</ul>
<h3><span data-text-color="primary">The role of digital tools in inclusivity</span></h3>
<p>Digital tools, including e-portfolios, can make a modest but potentially valuable contribution to promoting inclusivity in medical education. For example, we see in studies on biasing and the transfer of graduated doctors to the practical work environment as residents (learner handovers), the influence of negative feedback has a major impact on how the learner continues the learning path. A student who carries negative feedback consistently receives lower grades. And often this happens unconsciously or at least out of sight<span class="whitespace-nowrap">.</span></p>
<p>A number of very practical tools to promote diversity are:</p>
<ul class="marker:text-textOff list-disc pl-8">
<li>Personalized learning: E-portfolios allow students to follow their own learning path and reflect on their experiences. This can help recognize and value the unique perspectives and backgrounds of students.</li>
<li>Equal opportunities for feedback: Digital platforms can provide a more structured and objective way of giving feedback, reducing the chance of unconscious biases.</li>
<li>Accessibility: Online tools can increase the accessibility of education for students with physical limitations or other challenges.</li>
<li>Cultural competence: E-portfolios can contain specific modules aimed at developing cultural competence, better preparing students to work with diverse patient populations.</li>
</ul>
<h3><span data-text-color="primary">Conclusion</span></h3>
<p>Although digital tools such as e-portfolios may contribute more positively to more inclusive medical education than we think, they are only part of the solution. Real change requires a holistic approach, where all aspects of the training are critically examined and adjusted. Instruments or concepts such as &#8216;workplace-based assessment&#8217;, programmatic assessment, multi-source feedback, OSCE, where the origin of a decision can always be clear for the learner and the assessor, can be very useful in this regard. That&#8217;s why we believe these are also important elements of a good digital portfolio. Especially now that algorithms and AI support are increasingly being applied in such tools<span class="whitespace-nowrap">.</span></p>
<p>Creating an inclusive learning environment in medical education is not only a matter of justice for students but also an investment in better healthcare for everyone. By embracing diversity and promoting inclusivity, we train doctors who are better able to provide care to an increasingly diverse society<span class="whitespace-nowrap">.</span></p>

<p>In recent years, the demand for personalized and flexible education in higher education has grown rapidly. Both universities and universities of applied sciences are rethinking how students learn and show their progress.</p>
<p>Traditional portfolios, where students simply upload assignments, are no longer enough. Many programs now need a new kind of portfolio — one that supports continuous learning, reflection, and feedback. Several studies confirm this growing trend in education (1,2).</p>
<h3><span data-text-color="primary">Shift in learning</span></h3>
<p>Another major shift is the focus on workplace and practical learning. Learning doesn’t just happen in classrooms anymore. It happens on the job, during projects, and in internships.</p>
<p>Technical programs, for example, often use challenge-based learning: students work on real-world problems and learn by doing. This approach has already proven successful in fields like healthcare and business (2).</p>
<h3><span data-text-color="primary">Personalized learing</span></h3>
<p>Experts agree that personalized learning is key to meeting each student’s needs, talents, and learning style. A well-designed digital portfolio can make this possible — but only if it’s built the right way.</p>
<p>Not every digital portfolio supports personalized learning effectively. Institutions face several challenges when selecting and implementing such systems (1,4).</p>

<p>This is especially important in universities of applied sciences, where internships and hands-on projects are central to learning. Companies and institutions need simple, secure access to view and assess students’ portfolios.</p>
<p>In academic education, external access is just as relevant. Examination boards, research partners, and even potential employers should be able to review a student’s portfolio. It serves as a digital business card that showcases skills, growth, and achievements.</p>
<h3><span data-text-color="primary">User friendly and secure</span></h3>
<p>For this to work, a portfolio platform must offer user-friendly access for external assessors—without creating extra workload for the institution. Some programs work with hundreds of external evaluators, so managing access must be effortless and scalable.</p>
<p>At the same time, access must remain secure and traceable. The system should clearly record who provided feedback or completed an assessment. This ensures both accountability and data security.</p>

<p><span style="font-size: 14.4px;">A digital portfolio must fit seamlessly with the program’s learning and assessment vision. It should reflect how students learn, develop, and are evaluated — both within the institution and in the professional field.</span></p>
<p>In medical education, for example, many programs already use programmatic assessment supported by e-portfolios. For these programs, a digital portfolio designed specifically for this approach is essential.</p>
<h3><span data-text-color="primary">Multiple frameworks</span></h3>
<p>Other programs still rely on more traditional assessment methods. For them, a portfolio may serve a different purpose — for instance, as a developmental tool alongside standard exams and evaluations.</p>
<p>An effective portfolio should therefore adapt to different educational models. Whether a program is built around learning outcomes, competencies, or Entrustable Professional Activities (EPAs), the portfolio must support each approach with equal ease.</p>
<p>Ideally, a modern portfolio system can even combine multiple educational frameworks at once. This flexibility allows institutions to evolve their learning and assessment strategies without needing to change systems.</p>
<h3><span data-text-color="primary">Successful implementation and adoption</span></h3>
<p>A digital portfolio can only make an impact if it is successfully implemented and adopted by teachers and students. This requires clear planning, training, and ongoing support. Both groups need time to get familiar with the new learning approach and digital environment — a process that often proves challenging in practice.</p>
<p>Research shows that a step-by-step rollout, combined with strong guidance, greatly increases the chances of success. Involving teachers and students early in the process also helps build ownership and engagement (1,4).</p>
<p>Close collaboration between the institution and the supplier is essential. The supplier’s understanding of educational design plays a key role in translating the program’s learning and assessment model into an effective portfolio setup.</p>
<p>From our experience, introducing a digital portfolio is not just a technical project — it’s a change management process. The right expertise and experience with implementing portfolios in complex educational environments can make all the difference.</p>

<p data-start="236" data-end="499">A digital portfolio should never stand alone. It must be <strong data-start="293" data-end="318">seamlessly integrated</strong> with the institution’s existing <strong data-start="351" data-end="383">digital learning environment</strong>. Students and teachers need a single, connected ecosystem where learning, assessment, and feedback come together.</p>
<p data-start="501" data-end="748">Smooth integration with <strong data-start="525" data-end="599">learning management systems, scheduling tools, and assessment software</strong> is essential. Only when these systems work together can the portfolio truly function as a powerful instrument for <strong data-start="714" data-end="739">personalized learning</strong> (2,3).</p>
<p data-start="750" data-end="1079">That may sound obvious — and it is — but in practice, integration often falls short. Sometimes systems simply can’t connect. As a result, data ends up scattered across platforms, making it harder to get a complete picture of a student’s progress. When that happens, students may not receive the right support at the right time.</p>
<h3><span data-text-color="primary">What functionalities are needed for personalized learning?</span></h3>
<p>Research has shown that the chances of success increase significantly if certain conditions are met:</p>
<ol>
<li><strong>Adjusting learning goals and activities</strong><br />Teachers can adapt learning goals and associated learning activities per student in the portfolio to their specific level, interest, and learning needs.</li>
<li><strong>Formative evaluation and tailored feedback</strong><br />By closely monitoring students&#8217; portfolio submissions and progress, teachers can provide targeted, personal feedback that aligns with each individual student&#8217;s learning needs.</li>
<li><strong>Choices for students<br /></strong>A powerful aspect is that students make choices within the curriculum in their learning activities, work forms, and ways of learning.</li>
<li><strong>Flexible learning routes and paths</strong><br />With the flexibility of a digital portfolio, universities can easily set up different learning paths and choice trajectories, tailored to the preferences, talents, ambitions, and learning speeds of individual students.</li>
<li><strong>Live monitoring and adjustment</strong><br />The clear dashboard gives teachers current insight into the progress, development, and bottlenecks of each individual student.</li>
<li><strong>Individual timing and location of learning activities</strong><br />In addition, in our own practice and especially in medical education, we see the importance of being able to do learning activities individually in time and place as opposed to classical classroom learning. After all, in practice, you don&#8217;t often have a say in what will happen. And certainly in a healthcare facility or hospital this is the case. So for setting up the portfolio, it is then necessary that the actions or assignments with the corresponding feedback or assessment form can be easily changed by the institution.</li>
<li><strong>Flexibility at the process level</strong><br />Finally, in order to facilitate all of the above points at the process level, you must also be able to be very flexible. For example, the programmatic testing concept works with many different forms such as low-stake and high stake assessments, self-reflections, group feedback, 360 degree feedback and many other processes. This is because the specific learning situation requires a specific feedback form, content and process.</li>
</ol>
<h3><span data-text-color="primary">Conclusion</span></h3>
<p>Through these extensive functionalities for differentiation and personalization, an advanced digital portfolio makes it possible to shape higher education around the individual student. Students gain more control over their own learning process and can follow a learning path that seamlessly aligns with their personal needs, possibilities, and ambitions.</p>
<p>Thus, a well-implemented digital portfolio optimally supports universities in realizing personalized learning. A promising development for high-quality and future-oriented higher education, in which customization and talent development are central.</p>
